Academic accommodation requests require supporting documentation from an educational or medical record, report, or assessment provided by a health care provider, school psychologist, teacher, or educational institution.
Applications submitted without this documentation will be put on hold, and student-completed forms will not be reviewed.
Types of Documentation for Housing Requests
Students Without Documentation of a Disability:
